Based on the Logs you shared with us the Intel® SSDs DC S4500 Series are not reporting any kind of SMART errors/alerts. In the other hand, the SSU log is not showing your SSDs as detected, this can be due to the usage of a RAID controller as your LSI*3008 and the OS compatibility.
We advise you to check with the RAID controller manufacturer for updates of the firmware and compatibility.
